GP Referral Form

• Medicare rebates are available for eligible hearing assessments when you have the appropriate GP referral. Simply give this form to your GP to complete.
• One referral per appointment. Each referral covers a single appointment - later assessments will need a new form from your GP.
• Microsuction wax removal is not covered under Medicare ($99 per session). A courtesy discount applies when a full hearing assessment is performed at the same visit.
